PEER REVIEWED

This flowchart shows the screening process, and inclusion and exclusion for the reviews in this subanalysis of an umbrella review of determinants of digital health tool implementation in chronic disease prevention and management in clinical practice settings 2013 to 2015. The initial and updated search yielded 5,839 records. After duplicates were removed, the research team screened 5,327 titles and abstracts to result in 472 reviews for full text screening. Of these reviews, 362 reviews were excluded based on study language not being in English, full text not being available, incorrect intervention type, not a health care or clinical setting, incorrect study design, incorrect digital health tool recipient, no focus on implementation barriers and facilitators, implementer not being a health care provider, and insufficient methods. This process left 110 reviews, 95 of which were excluded for not focusing on chronic disease prevention and management. The final sample consisted of 15 reviews.
