For Jurisdiction Users: Clinics Offering School-Located Vaccines and Vaccines for Children

Jurisdiction POCs can add clinics offering School-Located Vaccines (SLV) and Vaccines for Children (VFC) in their jurisdiction. Note: Third-Party Clinics cannot be set up to offer SLV/VFC.

Jurisdictions under VAMS can add and bulk upload SLV Clinics, where recipients may be eligible for VFC. The VFC Program works to provide vaccines to children whose parents or guardians may not be able to afford them, ensuring that all children have a better chance of getting their recommended vaccinations on schedule.

Setting up Clinic offering SLV/VFC

  • Select the Manage Clinic tab and select the Add Clinic Button.
  • Enter the Clinic Point of Contact Information as requested, then click Next to proceed.
  • Enter the Clinic Information as requested.
    • NOTE: If you select Third-Party as the Clinic Type, you will not be able to set up the clinic for SLV/VFC services.
  • Under Clinic Services, select one of the following options from the drop-down menu:
    • School-Located Vaccines
    • Field-Located Vaccines
    • Vaccines for Children
    • Other
      • If Other is selected, briefly describe the clinic service in the space provided.
  • Enter remaining requested information and click
  • Review the information provided for Clinic Point of Contact and Clinic Information to ensure it is accurate and select
  • VAMS will display a confirmation message stating that your clinic has been created in the system. The clinic point of contact will receive an email to register and activate the clinic.
  • Clinic Administrators should follow existing steps to proceed with clinic registration in VAMS.
  • Once registered, VAMS will display a message confirming the clinic has been activated and direct you to complete set-up by defining clinic operating hours and days, appointment durations, treatment stations, and by enabling scheduling at that clinic location.
  • Select the Manage Clinic tab and select the Import Clinics Button.
  • Click the Clinic Import Template link in the pop-up window that appears. After clicking the link, the template file will open on your computer.
  • Fill out the Clinic Point of Contact and General Clinic Information as requested.
    • For the Clinic Services column, select one of the following options from the drop-down menu:
      • School-Located Vaccines
      • Field-Located Vaccines
      • Vaccines for Children
      • Other
        • If Other is selected, briefly describe the clinic service in the Other Clinic Service column.
    • NOTE: If Third-Party is selected as a clinic type, you will receive an error message indicating that the clinic cannot be set up as an SLV/VFC clinic, as the SLV/VFC clinic services are not available for third-party clinics.
  • Save the file as a CSV UTF-8 (Comma delimited) (.csv). You can find this option under the Save As drop-down menu in Excel.
  • Click Upload Files in the Import Clinic List pop- up window. You can also drag and drop your clinic list in the Drop Files area of the page.
  • Click Close.
