Evidence Summary: Control Asthma
CDC’s 6|18 Initiative includes a payer intervention to expand access to home visits by licensed professionals or qualified lay health workers to improve asthma self-management education and reduce home asthma triggers. This intervention, based on the 2007 National Asthma Education and Prevention Program  guidelines, expands access for individuals whose asthma is not well-controlled with medical management and intensive self-management education. Evidence cited references the engagement of CHWs to provide asthma self-management education as well as to decrease exposure to indoor asthma triggers.

Black Corals: A Gem of a Cancer Screening Program in South Carolina
In rural South Carolina, where many African American women have limited access to lifesaving medical screenings, the St. James-Santee Family Health Center launched Black Corals to increase cancer screening among women. The Community Guide served as a resource to help the Black Corals program dramatically increase breast and cervical cancer screenings in their community. Activities included promoted events and educational sessions in local churches, hair salons, and restaurants throughout the community.
