A Framework for Mobilizing Health Care to Respond to the Community Within the COVID-19 Pandemic

The 4 key outreach representatives are community partners, health system collaborators, academic collaborators, and community leaders. These are connected in a circle as elements of the outreach collaborative. Community partners are patients/family, faith-based, government/civic, education, health care, public health, nonprofit organization, and population of focus representative. Health system collaborators are board of trustees, president/CEO, senior leaders, quality and data analytics, innovation, front-line clinical team, and marketing and communications. Academic collaborators are dean’s office, school of medicine, school of nursing, school of public health, affiliated school of pharmacy, and research. Community leaders are faith-based, education, government/civic, health care, public health, nonprofit organization, and population of focus representative.
