A Public Health Framework to Improve Population Health Through Health Care and Community Clinical Linkages: The ASTHO/CDC Heart Disease and Stroke Prevention Learning Collaborative

Through the Heart Disease and Stroke Prevention Learning Collaborative, ASTHO, with the support of CDC, supported jurisdictions in their efforts to increase the percentage of adults whose blood pressure is under control. The logic model details the partnerships and information shared throughout the Learning Collaborative from 2013–2018, which led to improved data sharing and analysis, clinical to community linkages, care coordination, and financing mechanisms in support of increased identification, control, and management of high blood pressure.
