39. Is infant living at time of the report?

Check “yes” if the infant is living.

Check “yes” if the infant has already been discharged to home care.

Check “no” if it is known that the infant has died. If the infant has died, make sure that a death certificate is filed.

If the infant was transferred and the status is known, indicate the known status.
