Adult Tobacco Use Questions: List of Questionnaires

The National Health Interview Survey (NHIS) is comprised of several questionnaires. Prior to 1997, the NHIS consisted of a family core questionnaire and selected supplements. For data years 1997-2017, the NHIS consists of Family, Sample Adult, Sample Child core questionnaires, and supplements that either stand alone or are embedded in the core questionnaire. Below are the names of the NHIS core and supplement questionnaires that contain adult tobacco use questions.

Summary documents showing only the adult tobacco use questions are available for data years 1965-1995 and 1997-2011. These summaries were discontinued in 2012. The questionnaires for ALL data years with tobacco questions can be accessed via the Guide for Data Users. Questionnaires containing tobacco questions for 1997 forward also can be accessed from the list below. For tobacco questions asked in only a single year, the PDF page numbers of the questions are shown. For tobacco questions that were asked across multiple years, a link is provided only to the first data year. Because PDF page numbers change across years, the questionnaire number of the first tobacco question in the set (which remains the same across years) is provided to facilitate locating them in the pdf document. Questions were asked of adults aged 18 years and over unless otherwise noted.
