Evaluation of a Hypertension Surveillance System, Kenema Government Hospital, Sierra Leone, 2021

Data reporting begins when patients visit the triage point in the Kenema Government Hospital. Patient data are recorded in the chart. Patient records are tallied every month using the summary form and submitted to the KGH monitoring and evaluation officer by the fifth of every month. The monitoring and evaluation officer in the hospital submits DHIS2 records to the national level, the Directorate of Non-Communicable Diseases. Supervision and feedback are top-down, while data reporting is bottom-up.
