Geographic Patterns: Heart Disease Hospitalization Rates for 2018 through 2020 for Adults Aged 65 Years and Older by County. The map shows that concentrations of counties with the highest heart disease hospitalization rates – meaning the top quintile – are located primarily in Michigan, Louisiana, Kentucky, West Virginia, Pennsylvania, Ohio, Arkansas, Mississippi, Alabama, Georgia, North Carolina and Tennessee. Pockets of high-rate counties also were found in South Carolina, Florida, Indiana, Illinois, Missouri, New Jersey, Oklahoma, and Texas.
