Your application is currently being reviewed to validate that you are an employee of a state, tribal, local, or territorial (STLT) public health department. Once approved, you will receive an email with your user name and temporary password.

Next Steps

Once we have verified that you are a STLT public health department employee, you will:

  1. Receive an email that includes a link, user name, and temporary password to access the STLT Collaboration Space through the CDC SAMS system. Follow directions in that email message to log into CDC SAMS to activate your account.
  2. Once your account is activated, you will receive an email informing you that you now have access to STLT Collaboration Space.
