- Your personal information in V-safe is protected and confidential. You can sign up for V-safe and complete check-ins for someone you care for (such as, a dependent).
- If you signed up for V-safe before October 2023, you need to create a new account to share your experience with a COVID-19 or RSV vaccine.
Signing up for V-safe
- Using your computer, tablet or smartphone, visit the V-safe sign-up page.
- You can also scan the QR code located on the vaccine information sheets using your tablet or smartphone camera.
- Select Register Now and fill in the required fields to verify you do not have an existing account.
- If V-safe does not detect an existing account, select Register.
- If V-safe detects an existing account, enter your username and password to log into your account.
- Answer the account sign-up questions including demographics and contact information. Select Submit.
- You must sign up for an account for yourself before participating in V-safe for someone else.
- V-safe will send you a verification code. Enter the verification code and select Submit. A message will appear on the screen confirming that your account has been successfully created.
- V-safe will send you a follow-up link. Click and follow the instructions to activate your account.
- Follow instructions on the screen to create a password. Log into your account using the new password.
- Follow the next series of instructions to add a new vaccine to your V-safe account.
Telling us about the vaccine(s) you received
- Log in to V-safe.
- Select the Start button or Add a Vaccine tab.
- In the drop-down box, start typing the kind of vaccine (such as RSV, COVID-19, Influenza), select the vaccine you received, and select 'Add to list.'
- Enter the relevant Date and Manufacturer or Brand for the vaccine you received. You may enter up to 5 vaccines at one time. When finished adding all vaccines, select 'Next.'
- If one of the vaccines received is eligible for follow-up (for example, COVID-19 and RSV), answer the initial health check-in questions. Take time to review and confirm that the information is correct before selecting Submit. This will be the only opportunity to modify your entry.
- If you enroll before 12 p.m. local time, V-safe will send you an email or text to start your first check-in around 12 p.m. the same day. If you enroll after 12 p.m., V-safe will allow you to answer questions immediately and V-safe will send you a link that can be used later — just follow the instructions.
- Check-in links expire. Try to complete your check-in promptly after receiving your reminder.
Adding a dependent
After you sign up for V-safe, you can add a person you care for (such as, a dependent) from the ‘Home’ or ‘My Account’ tab. These instructions can help you complete check-ins for someone else if you are their parent or caregiver.
Information you will need
It is helpful to have your dependent's vaccination information available to enter details about which vaccine(s) they received and the date received. If this information is not available, please contact your vaccination provider.
- Log in to V-safe.
- Select Add Dependent from the Home or My Account tabs .
- Enter your dependent's demographic information (name, birth date, etc.) and select Submit.
You will now see dependents on the 'User' dropdown for each dependent at the top left of the screen on the Home tab. You can also see your registered dependent(s) by selecting the My Account tab.
Sharing your dependent's vaccine(s)
- Log in to V-safe.
- On the Home tab, select your dependent's name from the dropdown list (located to the left of or below your name in the upper left corner).
- Select the Start button or Add a Vaccine tab.
- In the drop-down box, start typing the kind of vaccine (such as, RSV, COVID-19, Influenza), select the vaccine you received, and select 'Add to list.'
- Enter the relevant Date and Manufacturer or Brand for the vaccine you received. You may enter up to 5 vaccines at one time. When finished adding vaccines, select 'Next.'

Adding follow-up vaccine doses
To add a follow-up vaccine dose, follow the instructions for telling us about the vaccine(s) you received.
Completing V-safe check-ins
- V-safe will send reminders to complete regular check-ins for yourself or someone you care for (such as a dependent) to your preferred method of contact.
- Check-in links expire. Try to complete your check-in promptly after receiving your reminder. Once a check-in has expired, it is no longer available for completion.
- Click and follow instructions to answer questions (about you or your dependent) when you're ready. Check-ins should take less than two minutes to complete and must be completed within the geographic area of the United States and its territories.
- Take time to review and confirm that your survey responses are correct before selecting Submit. This will be the only opportunity to modify your response.
- V-safe will send check-in reminders for up to six weeks, 42 days (or more, for maternal RSV vaccine recipients). You can opt out at any time.
Additional questions you may have
V-safe allows you to report to the Vaccine Adverse Event Reporting System (VAERS). If you report seeking medical care after vaccination, V-safe will ask you to complete a VAERS report describing your side effects or health problems. Information you provide in V-safe can be used to start your VAERS report, and if you choose to start a report, some of your V-safe information (such as your name) will automatically be added. Before submitting your VAERS report, you will be able to review it and add more details about your vaccination experience.
