Aligning to a shared vision
CSD provides a framework and shared vision for public health data exchange. CSD supports public health professionals in planning, designing, and implementing solutions for data modernization.
The CSD package defines people, processes, and technologies needed to achieve CSD’s future vision. Co-created with public health professionals, the package accelerates modernization, strengthens collaboration, and builds trust. CSD case data insights help public health professionals access information to protect communities nationwide.
Using the CSD Package
The CSD package includes:
- Executive Summary: Overview of CSD artifacts and guidance for use
- Future State Story: Articulates a shared vision for public health data exchange
- Future State Overview: Defines jobs and conditions to achieve the future state
- Year One Discovery: Documents practitioner roles, pain points, and constraints across public health

Putting People First
Using human-centered design, CDC’s CSD team engaged public health co‑creators to develop the package including:
- Internal interviews with CDC program practitioners
- External interviews with jurisdiction practitioners
- Focus groups with health systems and providers
- Ongoing feedback from national public health partners and federal partners
- A series of in-person workshops where select public health professionals within each group will design the future state together.
Collecting Insights to Inform Solutions
Site Visits
The CSD team conducted 25 site visits to state, tribal, and local health departments between March and October 2024.
CSD Summit
In August 2024, CDC convened a summit of over 100 epidemiologists and data scientists from 25 state, local, and tribal jurisdictions, 11 public health partner organizations, and seven CDC centers and institutes. National partners included the Council of State and Territorial Epidemiologists, National Association of County and City Health Officials, Association of Public Health Laboratories, and the Big Cities Health Coalition.

During the sessions, participants listened to stories, formed connections, and generated ideas. Each session built on shared insights and experiences to create a vision for a future for case‑based surveillance.
