NWS Case Report Form

1. Ensure your jurisdiction has access to the 1 CDC Data Platform (1CDP)

  • To obtain access to 1CDP, the state epidemiologist or designee should email newworldscrewworm@cdc.gov with the subject line "NWS 1CDP Access."
  • Include the person's name, email address, role (job title or role for NWS in your jurisdiction), and their 1CDP status (if they already have access to 1CDP or not).

2. Use the case report form in 1CDP to notify CDC of human NWS cases.

  • Submit the case report form to CDC by direct entry into 1CDP or through a CSV bulk upload in 1CDP.
  • A fillable PDF version of the NWS case report form is available to aid in data collection, particularly in areas where internet access may be limited. However, jurisdictions should use the case report form in 1CDP to submit the data to CDC.

Confirmatory testing

CDC's Diagnostic Parasitology Laboratory, DPDx, provides confirmatory testing for NWS by telediagnosis and specimen submission. Contact DPDx for specimen or telediagnosis submission instructions at dpdx@cdc.gov.
