Public health materials have often used the phrase "elevated blood lead level" to identify children with more lead in their blood than most children have. But "elevated" implies there is some lower, acceptable level. There is not.

No safe blood lead level in children has been identified. Even low levels are linked to developmental delays, learning difficulties, and behavioral problems.

Our language should reflect that.

What science shows

Lead can affect nearly every organ system. Young children, whose brain and nervous system are still developing, are especially susceptible to the adverse effects of lead.

CDC, the World Health Organization, and the National Toxicology Program all agree: there is no known level of lead exposure without harmful effects. Research has found that blood lead levels far below what was once considered a level of concern can still affect learning and behavior.

The CDC blood lead reference value (BLRV) is not a safety threshold. It identifies children whose levels are higher than most children's so follow-up can happen quickly. When a child's blood test result is below the BLRV that doesn't mean that the child had no exposure to lead or no risk of negative health effects.

Why we avoid the term "elevated"

In everyday health communication, the term "elevated" often implies a scale with a safe range, a caution range, and a high range. That framing may work for some health measures, but it does not work for lead.

When parents hear that a child's blood lead level is "not elevated," they may reasonably conclude there is no risk. When parents hear "elevated," they may focus only on the highest measured levels rather than on the broader prevention message: no lead exposure is best.

Plainer alternatives work better:

  • "A blood lead level that may harm health"

What parents should know

Most children with lead exposure do not look or act sick right away. A blood lead test is the best way to know whether a child is exposed.

Children under 6 are at greatest risk. Their brains and bodies are still developing, and they are more likely to put hands and objects in their mouths.

Lead can be found in older paint, dust, soil, drinking water, and some imported products such as toys, jewelry, candies, or traditional home remedies. Also, certain jobs or hobbies such as home renovations, casting, or soldering can bring lead into the home. Learning where lead may be present — and taking steps to limit exposure — is the most important thing a parent or caregiver can do.

Partners can help by using language that is clear, accurate, and focused on prevention.

Recommended phrases include:

  • "Even low levels of lead can harm a child's health."
  • "The blood lead reference value is not a safety threshold."

Partners can also explain why CDC uses a reference value. The BLRV helps identify children whose blood lead levels are higher than most children's levels. A result below the BLRV does not mean there was no exposure or no risk.

Consistent language across agencies, health care providers, and community partners can help reduce confusion and support earlier action.

We avoid the term "elevated" because it can unintentionally suggest that some blood lead levels are safe. There is no scientific basis for that idea.
