HAI/AR and Accreditation Crosswalk Toolkit

Key points

  • The HAI/AR and Accreditation Crosswalk toolkit helps health department staff understand connections between HAI/AR program activities and PHAB accreditation.
  • Originally published in 2019, the 2023 update includes condensed content and uses the PHAB Version 2022 Standards & Measures.

About this toolkit

CDC's Division of Healthcare Quality Promotion and the Public Health Infrastructure Center (PHIC), with input from health departments and Public Health Accreditation Board (PHAB), created a crosswalk to highlight links between HAI/AR Program activities and PHAB's accreditation standards and measures for health departments.

In 2023, CDC updated the HAI/AR and Accreditation CrosswalkA to streamline content and reflect updates to HAI/AR program guidance and the Version 2022 PHAB Standards and Measures.

How to use this toolkit

HAI/AR and health department staff can use the HAI/AR and Accreditation Crosswalk Toolkit to:

  • Foster engagement.
  • Identify opportunities for HAI/AR documentation or activities that support accreditation requirements.
  • Pursue PHAB accreditation and reaccreditation.
  • Improve crosscutting performance.

What are users saying‎

"The crosswalk reiterates that everybody in this agency has a part in accreditation." – State Health Department Accreditation Coordinator

If you are in the accreditation process using Version 1.5 of the PHAB Standards and Measures, the 2019 HAI/AR Accreditation Crosswalk can be downloaded here. You can find general information about accreditation here.

What's included

The toolkit divides content by PHAB domains, context and examples of HAI/AR activities for each PHAB standard, and opportunities to connect activities with accreditation efforts. The 2023 update also includes:

  • Condensed content: Standard level examples.
  • Examples for a broader audience: Relevant to any health department HAI/AR program regardless of funding.
Table visualizing stronger and weaker connections between PHAB Domains and HAI/AR Programs, covered in the Accreditation Crosswalk.
While there are potential connections across almost all PHAB Domains, PHAB Domains 2 and 8 may contain the most relevant and robust opportunities to demonstrate where HAI/AR activities can contribute to accreditation documentation requirements. The table to the left provides a snapshot of areas for collaboration and coordination opportunities. The black dots show which HAI/AR program topics have stronger connections to certain PHAB domains while the white dots show weaker connections.
  1. The HAI/AR Accreditation Crosswalk is a supplemental tool and not intended to provide all definitive linkages between PHAB standards and HAI/AR program activities. Completion of the examples provided does not guarantee conformity to PHAB documentation requirements.
