TB-Specific Evaluation Plan Examples
The following are examples of TB-specific evaluation plans that were created to assist programs in developing their own plans; these are examples only and are not intended to be duplicated.

• Identifying Factors Associated with Completion of Latent TB Infection (LTBI) Treatment
  This evaluation plan focuses on identifying facilitators, barriers, and potential solutions to barriers to completing treatment among patients with LTBI.
• Completion of Treatment for TB Disease by Using Incentives and Enablers
  This evaluation plan explores using specific strategies for increasing completion of treatment among patients with TB disease. This plan includes goals aligned with national TB program objectives and targets.
• Increase Use of Nucleic Acid Amplification Tests (NAAT) at Local Health Jurisdictions to Improve Treatment Initiation Performance
  This evaluation plan focuses on increasing NAAT results among sputum smear-positive, sputum smear-negative, and HIV-positive TB patients with NAAT result reported prior to culture results.  The plan illustrates the use of Specific, Measurable, Achievable, Relevant and Time-bound (SMART) objectives.
• Tuberculosis Infection Identification Among Persons with Diabetes Mellitus in Community Health Centers
  This evaluation plan focuses on collaborating with Community Health Centers to improve TB infection identification among individuals with diabetes mellitus.
