How Do We Apply?
The 2020 application period for PHAP host sites is now closed. The next application cycle will open in early 2021.
PHAP accepts host site applications through the Electronic Fellowship Management Systemexternal icon (eFMS) v2.0 during the application period only. All applicants must “Sign Up” to register as a new user. Current host site supervisors are pre-populated into the host site supervisor database; please submit an eFMS HelpDesk ticket hereexternal icon for changes to the information provided by your associate. Please do not use previous links for eFMS v1.0 as those submissions will not be considered. For those who submitted an application last year, please contact PHAP@cdc.gov for access to that host site application.
Once a host site has been selected, PHAP provides a copy of the Host Site Agreement to Detail via email. A signed copy of this document must be returned to PHAP prior to the associate’s start date.
Please note: the dates below provide an example timeline and are subject to change.
|February||Guidelines for the components of an application package will be made available.|
|March||Brief training on the eFMS system will be made available.|
|April||The host site application period is open from April 1–21, 2020. Applicants may be contacted for additional information throughout the host site application review process.|
|May-June||Host site assignments are chosen and then matched with associates.|
|July||PHAP notifies host sites and associates about their matches.|
|Early October||Associates begin their host site assignments.|
|November||Associates attend PHAP Orientation at CDC Headquarters in Atlanta, Georgia.|
