This Alert describes seven accidents that resulted in the deaths of seven workers who were operating forklifts or working near them. In each accident, the deaths could have been prevented if the procedures had been followed and the appropriate safety equipment had been used, and if the provisions established by OSHA standards had been followed. This publication supersedes 2000-112.
