Integrated HIV Prevention and Care Plan

Integrated HIV Prevention and Care Plan Guidance

To support Centers for Disease Control and Prevention (CDC) and Ryan White HIV/AIDS Part A, Part B HIV planning bodies and grantees, and with accelerating progress toward reaching the goals of the National HIV/AIDS Strategy, the CDC Division of HIV/AIDS Prevention (DHAP) and the Health Resources and Services Administration (HRSA), HIV/AIDS Bureau (HAB) have developed a guidance document for the submission of an Integrated HIV Prevention and Care Plan, including the Statewide Coordinated Statement of Need (SCSN).

HIV Planning Guidance (HPG)

The purpose of the HIV Planning Guidance is to provide CDC grantees a blueprint for the HIV planning process and flexibility to design and implement a participatory HIV planning process. This Guidance supports health departments in the implementation of HIV prevention programs supported by Funding Opportunity Announcement PS12-1201. The HIV Planning Guidance, formerly known as the HIV Prevention Community Planning Guidance, is available below.

Note: Funded jurisdictions should establish and maintain an HIV planning group (HPG) and a process that entails engaging partners and stakeholders in prevention and care planning.
