 I.         Funding Opportunity Description

Purpose: This supplement supports Part A (HIV Prevention Integration) of Program Announcement (PA) #04073. The purpose of the supplement is to develop the capacity of primary health care settings serving American Indian/Alaska Natives (AI/AN) to effectively integrate HIV prevention services. This will be accomplished by providing HIV integration training and technical assistance to organizations implementing  HIV integration efforts in Indian Country, including but not limited to other Regional Training Centers (RTCs); Indian Health Service (IHS), tribal health, and urban Indian health programs (I/T/U); other publicly-funded primary health care settings serving AI/AN; and other relevant non-profit institutions and agencies, such as Tribal Epidemiology Centers and Indian health boards. The training curriculum and technical assistance package to be used was adapted for use among AI/AN by the awardee for the FY 2007 supplement to #04073, Integrating HIV and Other Prevention Services into Reproductive Health and Community Settings in Indian Country.

Activities:

Awardee activities for this program are as follows:

• Market adapted HIV integration training to potential participants who support HIV integration efforts in Indian Country, including but not limited to other Regional Training Centers (RTCs); Indian Health Service (IHS), tribal health, and urban Indian health programs (I/T/U); other publicly-funded primary health care settings serving AI/AN; and other relevant non-profit institutions and agencies, such as Tribal Epidemiology Centers and Indian health boards
• Deliver one HIV integration training events in at least three HHS regions in the UnitedStates.
• Provide technical assistance to integrate HIV at the clinic level to at least two primary health care settings serving AI/AN that participated in any of the HIV integration trainings.
• Evaluate each training event and all technical assistance efforts to ensure that each training event increased awareness about the adapted training curriculum and technical assistance package and that technical assistance efforts increased organizational capacity to integrate HIV prevention into reproductive health services.

In a cooperative agreement, CDC staff is substantially involved in the program activities, above and beyond routine grant monitoring.  CDC activities for this program are as follows:

• Assist in identifying potential participants and serve as liaison as necessary.
• Provide consultation to grantee concerning the delivery of the HIV integration training and technical assistance efforts.
• Provide electronic and hardcopy form of the training curriculum and technical assistance package adapted by awardee for the FY 2007 supplement to #04073, Integrating HIV and Other Prevention Services into Reproductive Health and Community Settings in Indian Country.

1.         Eligible Applicants: Eligible applicants that can apply for this funding opportunity are grantees currently funded through PA #04073.

2.         Other: If a funding amount greater than the ceiling of the award range is requested, the application will be considered non-responsive and will not be entered into the review process.  The applicant will be notified that the application did not meet the submission requirements.

3.         Special Requirements:

If the application is incomplete or non-responsive to the special requirements listed in this section, it will not be entered into the review process.  The applicant will be notified the application did not meet submission requirements.

• Applicants must demonstrate training or technical assistance collaboration(s) with an I/T/U in the past 3-5 years. This activity (ies) should be documented by a reference letter from the I/T/U in question.

  • Describe your organization’s previous training or technical assistance activity (ies) with one or more I/T/U(s) in the past 3 to 5 years. Include the name, location, and type of I/T/U; a description of staff trained (e.g., job category, demographic data); a synopsis of the training or technical assistance provided; a description of the curricula, tools, or other materials used; and any outcome or evaluation results. Include a reference letter from each of these I/T/U(s).
• Objectives
  • Define specific, measurable, achievable, realistic, and time-phased objectives for the project that are consistent with the overall purpose of PA #04073.
• Plan/Methods
  • Describe the strategy you will use to address the first three items listed above under Awardee Activities (i.e., “Market adapted HIV integration training to potential participants…” “Deliver one HIV integration training event...” and “Provide technical assistance to integrate HIV…”)
  • Describe anticipated obstacles and how you will overcome them.
  • Provide a timeline demonstrating the order and timing of key project activities as they relate to the proposed goals and objectives.
• Evaluation
  • Clearly identify an evaluation plan for both the training events and the technical assistance efforts that is consistent with the overall evaluation plan of PA #04073. The logic model to be used is the model in PA# 04073.Describe how an initial and final HIV integration checklist will be completed during the project year.
  • Provide process and outcome evaluation indicators for each measurable objective. At least one indicator will be related to desired capacities to be built.
  • Provide a data collection, analysis, and management plan.
  • Describe a plan to disseminate data findings and evaluation results to all PA# 04073 grantees and other primary health care settings serving AI/AN.
• Project Staff
  • Provide job descriptions for anticipated project staff including consultants and contractors that describe previous experience working with I/T/U and identify specific roles (e.g., project lead, management and supervision, planning, curricula development, training delivery, technical assistance, evaluation, staff support). Attach curriculum vitas of existing and newly proposed staff, consultants, and contractors as an appendix.

Applicants are required to provide measures of effectiveness that will demonstrate the accomplishment of the various identified objectives of the cooperative agreement.  Measures of effectiveness must relate to the performance goals stated in the “Purpose” section of this announcement.  Measures must be objective and quantitative and must measure the intended outcome.  The measures of effectiveness must be submitted with the application and will be an element of evaluation.

If requesting indirect costs in the budget, a copy of the indirect cost rate agreement is required.  If the indirect cost rate is a provisional rate, the agreement should be less than 12 months of age.

• Does the applicant adequately describe the strategy it will use to market the adapted HIV integration training and technical assistance package to potential participants, including primary health care settings serving AI/AN and how, if necessary, the applicant will provide travel scholarships to selected participants?
• Does the applicant adequately describe the strategy it will use to deliver at least one HIV integration training event in at least three HHS regions in the United States.
• Does the applicant adequately describe the strategy it will use to provide technical assistance to integrate HIV at the clinic level to at least two primary health care settings serving AI/AN that participated in the HIV integration training? Does the strategy include use of the HIV integration checklist, describe other assessments that might be used, and include a written plan for technical assistance?
• Does the applicant describe anticipated obstacles and how they will overcome them?
• Does the applicant provide a feasible timeline demonstrating the order and timing of key project activities as they relate to the proposed goals and objectives?

2. Review and Selection Process

Applications will be reviewed for completeness by the Procurement and Grants Office (PGO) staff, and for responsiveness jointly by the National Center for Chronic Disease Prevention and Health Promotion and PGO. Incomplete applications and applications that are non-responsive to the eligibility criteria will not advance through the review process.  Applicants will be notified the application did not meet submission requirements.

An objective review panel will evaluate complete and responsive applications according to the criteria listed in the “V.1.  Criteria” section above.

• Applications will be reviewed objectively by CDC professional staff from outside the funding centers (the National Center for Chronic Disease Prevention and Health Promotion).
• Each application will be reviewed by a primary, secondary, and tertiary reviewer.
• Responsive applications will be reviewed and then ranked by score according to the criteria described in this program announcement.
• The applicant with the highest score will be referred to the Director, NCCDPHP, who will make the final decision.
• All applicants will receive a written report summarizing the strengths and weaknesses of their application.

CDC will provide justification for any decision to fund out of rank order.

VI.       Award Administration Information

1. Award Notices

Successful applicants will receive a Notice of Award (NoA) from the CDC Procurement and Grants Office.  The NoA shall be the only binding, authorizing document between the recipient and CDC.  The NoA will be signed by an authorized Grants Management Officer and emailed to the program director and a hard copy mailed to the recipient fiscal officer identified in the application. Unsuccessful applicants will receive notification of the results of the application review by mail.

2. Reporting Requirements

The applicant must adhere to the reporting requirements established under Program Announcement Number 04073.
