Observation tools and Checklists
The Observation tools and checklists below are intended to promote CDC-recommended practices for infection prevention in hemodialysis facilities. The Observation tools and checklists can be used by individuals when assessing staff practices. They can also be used by facility staff themselves to help guide their practices.
Suggested Use of Dialysis Observation tools:
The Observation tools provided are meant to be part of a planned series of observations and measurements conducted within hemodialysis facilities. Consider implementing the tools to assess how well your staff is adhering to CDC-recommended practices. Observation tool results should be regularly reviewed with your staff to help promote desired practices and inform quality improvement projects.

Suggested Use of Dialysis Checklist Tools:
The checklists provided are meant to be used as a simple reference tool within hemodialysis facilities. Consider implementing the tools during orientation or training of newly hired staff. Subsequently, they may also be used as a resource during infection prevention education or as a posted reminder of recommended practices.
