Methods for State Oral Health Surveys that began data collection in 2013

Colorado

The survey was conducted during the 2013-2014 school year. The sampling frame consisted of all Head Start centers with 25 or more children. The sampling frame was stratified by urban/rural status of the county where the Head Start center was located. A systematic probability proportional to size sampling scheme was used to select 10 centers of which 9 agreed to participate. Of the 1388 children invited to participate, 739 were screened for a response rate of 53%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The data were weighted for the sampling scheme and adjusted for nonresponse.

Florida

The survey was completed during the 2013-2014 school year. The sampling frame consisted of all public elementary schools with more than 15 students in 3rd grade. With implicit stratification by region and percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic probability proportional to size sampling scheme was used to select 41 schools. Twenty-three dental professionals completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 5050 eligible students, 2061 were screened; for a response rate of 41%. In the state, 61% of public elementary school students participate in the FRL program and 61% of the participating students were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

Illinois

The survey was completed during the 2013-2014 school year. The sampling frame consisted of all public elementary schools with 15 or more students in 3rd grade.  With implicit stratification by region, urbanicity and percent of students eligible for the free/reduced price school lunch (FRL) program , a systematic probability proportional to size sampling scheme was used to select 100 schools of which 88 agreed to participate. Twenty-eight trained dentists and dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 8543 eligible students, 3772 were screened; for a response rate of 44%.  In the state, 54% of public elementary school students participate in the FRL program while 57% of the children screened were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

Montana

The survey was completed during the 2013-2014 and 2014-2015 school years. The sampling frame consisted of all public and Bureau of Indian Education elementary schools with 10 or more students in 3rd grade.  With implicit stratification by urban/rural status of the county and percent of students eligible for the free/reduced price school lunch (FRL) program , a systematic probability proportional to size sampling scheme was used to select 40 schools of which 40 agreed to participate. Trained dentists and dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 2897 eligible students, 1862 were screened; for a response rate of 64%.  In the state, 45% of public elementary school students participate in the FRL program while 47% of the children enrolled in the participating schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

New Hampshire

The survey was completed during the 2013-2014 school year. The sampling frame consisted of all public elementary schools with 15 or more students in 3rd grade. With stratification by county and implicit stratification by percent of students eligible for the free/reduced price school lunch (FRL) program , a systematic sampling scheme was used to select 118 schools of which 117 agreed to participate. Trained dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 6040 eligible students, 3199 were screened; for a response rate of 53%. In the state, 30% of public elementary school students participate in the FRL program while 36% of the students at the participating elementary schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

Rhode Island

The survey was completed during the 2013-2014 school year. The sampling frame consisted of all public elementary schools with 3rd grade. With stratification by urban/rural and implicit stratification by percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic sampling scheme was used to select 74 schools of which 60 agreed to participate. Trained dental professionals completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the 4272 eligible students, 2529 were screened; for a response rate of 59%. In the state, 50% of public elementary school students participate in the FRL program while 60% of the students at the participating elementary schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

South Dakota

The survey was conducted during the 2013-2014 school year. The sampling frame consisted of all public, private and Bureau of Indian Education elementary schools with 3 or more students in 3rd grade. With implicit stratification by urban/rural status of the county and percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic sampling scheme was used to select 40 schools with 3rd grade. Data is available for 36 of the 40 sampling intervals. Of the 2,219 3rd grade children in the 36 participating schools, 1053 participated for a response rate of 47%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. In the state, 44% of elementary schoolchildren are eligible for the FRL program; among participating schools 42% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.

Texas

The survey was completed during the 2013-2014 school year. The sampling frame consisted of all Texas Head Start grantees. A probability proportional to size (PPS) sampling scheme was used to select grantees within four strata, non-border urban, non-border rural, border urban and border rural. Selected grantees provided enrollment for each center and a PPS sampling scheme was used to select centers. Only those children that returned a positive consent form were screened. Of the 6799 Head Start children enrolled in the 61 selected sites, 3143 were screened; for a response rate of 46%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. The data were weighted for the sampling scheme and adjusted for nonresponse.

Vermont

The survey was conducted during the 2013-2014 school year. The sampling frame consisted of all public elementary schools with 7 or more students enrolled in 3rd grade. The sampling frame was stratified by region then ordered by percent of children participating in the free/reduced price school lunch (FRL) program. A systematic probability proportional to size sampling scheme was used to select 22 schools. Two of the selected schools did not have kindergarten so the two feeder schools with kindergarten were added to the sample. Kindergarten and 3rd grade children were screened using a combination of positive and passive consent and the statewide response rate was 76%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. In the state, 40% of elementary schoolchildren are eligible for the free/reduced lunch program (FRL); among students attending participating schools, 42% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.

West Virginia

The survey was conducted during the 2013-2014 school year. The sampling frame consisted of all public elementary schools with 15 or more students enrolled in 3rd grade. The sampling frame was stratified by health region then ordered by percent of children participating in the free/reduced price school lunch (FRL) program. A probability proportional to size sampling scheme was used to select 40 schools; all agreed to participate. Children were screened using passive consent and the statewide response rate was 93%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. In the state, 57% of elementary schoolchildren are eligible for the free/reduced lunch program (FRL); among students attending participating schools, 56% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.
