Disability and Health State Programs
CDC’s Disability and Health Promotion Branch currently funds 10 State Disability and Health Programs under the cooperative agreement CDC-RFA-DD21-2103: Improving the Health of People with Mobility Limitations and Intellectual/Developmental Disabilities through State-based Public Health Programs. The overall goal of this funding opportunity is to reduce health disparities experienced by adults with intellectual and developmental disabilities (IDD) and adults with mobility limitations (ML) in the United States. To help achieve this goal, state programs are funded to
Establish, expand, and enhance partnerships with organizations that serve adults with disabilities
Train healthcare personnel on best practices in providing accessible preventive health care to adults with disabilities
Link adults with IDD to preventive health care and health promotion programs in their community
Identify gaps in resources and tools to promote health among adults with disabilities and define action steps to address those gaps
Implement and evaluate evidence-based health behavior interventions, as well as policy, system, and environmental changes
Disseminate key findings and lessons learned
The following outcomes are anticipated at the end of the five-year funding period (2021-26):
- An increase in the number of healthcare providers that can offer accessible preventive health care to adults with disabilities
- Fewer unmet preventive health care needs, including mental health, among adults with disabilities
- Improved health behaviors and wellness among adults with disabilities
- Decreased rates of chronic conditions, and their associated risk factors, among adults with disabilities
