General Information
Philadelphia's IIS is called PhilaVax, and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
The jurisdiction is authorized by law to operate PhilaVax. For more information, please see City of Philadelphia Department of Public Health Regulations Governing the Immunization and Treatment of Newborns, Children and Adolescents § 6a and § 6b.
Child Consent to Participate
For children, PhilaVax requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.
Adult Consent to Participate
For adults, PhilaVax requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.
Provider Reporting
Philadelphia does have a mandate for vaccination providers to report immunizations to PhilaVax. The reporting mandate:

Data Sharing
Philadelphia does have a policy that regulates sharing data from PhilaVax with other public health entities. The data sharing policy:
