General Information
Indiana's IIS is the Children and Hoosier Immunization Registry Program (CHIRP), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.
The jurisdiction is authorized by law to operate CHIRP. For more information, please see Indiana Code § 16-38-5-1.
Child Consent to Participate
For children, CHIRP requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.
Adult Consent to Participate
For adults, CHIRP requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.
Provider Reporting
Indiana does have a mandate for vaccination providers to report immunizations to CHIRP. The reporting mandate:
- Applies only to a subset of vaccine recipients and
- Applies only to certain providers.
Data Sharing
Indiana does have a policy that regulates sharing data from CHIRP with other public health entities. The data sharing policy:
- Allows data to be shared upon request or with approval/authorization.
