What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Connecticut.

General Information

Connecticut's IIS is called CT WiZ, and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, CT WiZ requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

For adults, CT WiZ requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Connecticut does have a mandate for vaccination providers to report immunizations to CT WiZ. The reporting mandate:

  • Mandates universal reporting by law.

Data Sharing

Connecticut does have a policy that regulates sharing data from CT WiZ with other public health entities. The data sharing policy:

  • Requires a data use/sharing agreement or memorandum of understanding and
