What to know

Immunization information systems (IIS) are confidential, population-based, computerized databases that record all immunization doses administered by participating providers to persons residing within a given geopolitical area. This information details how to access immunization records for vaccine recipients who received a vaccine in Arizona.

General Information

Arizona's IIS is the Arizona State Immunization Information System (ASIIS), and it includes immunization records for vaccine recipients of all ages. Visit this webpage to learn more about the IIS.

Child Consent to Participate

For children, ASIIS requires implicit consent with opt-out by law. Children are included in the IIS unless their parent(s)/guardian(s) choose to opt them out.

For adults, ASIIS requires implicit consent with opt-out by law. Adults are included in the IIS unless they choose to opt out.

Provider Reporting

Arizona does have a mandate for vaccination providers to report immunizations to ASIIS. The reporting mandate:

  • Applies only to a subset of vaccine recipients and
  • Applies only to certain providers.

Data Sharing

Arizona does have a policy that regulates sharing data from ASIIS with other public health entities. The data sharing policy:
