Letter of Intent (LOI)

CDC strongly urges you to complete a LOI prior to the submission of your application. The purpose of this letter is to inform CDC of your interest in applying for funding under PS19-1904. Although a LOI is not required and non-binding, it is highly recommended as it allows CDC program staff to estimate the number of and plan for the review of submitted applications

The LOI should be prepared on letterhead and include the following information:

  • Number and title of this notice of funding opportunity
  • Descriptive title of proposed project
  • One selected funding component including related track and region for proposed project
  • Name, address, telephone number, and email address of the Principal Investigator/Project Director
  • Name, address, telephone number, and email address of the primary contact for writing and submitting this application

The LOI should be addressed to:

DaDera Moore, Project Officer
Department of Health and Human Services
Centers for Disease Control and Prevention
1600 Clifton Road, NE
Mailstop E-40
Atlanta, GA 30333
Telephone: (404) 718-3180
Email: cbanofo@cdc.gov

Completed LOI must be electronically submitted to cbanofo@cdc.gov

An email acknowledging CDC’s receipt of a LOI submission will be sent to the potential applicant.
