The Issue

  • In November 2024, the New York State Department of Health (NYSDOH) identified six patients in a western New York nursing home with highly drug-resistant infections caused by carbapenemase-producing Klebsiella pneumoniae. Genetic testing identified 18 additional, closely related isolates from the same region collected beginning in spring 2024.

What We Did

  • NYSDOH investigated to characterize the outbreak and limit transmission.

What We Found

  • We identified 91 cases from patients in 24 facilities. Assessments of four hospitals and 19 nursing homes identified gaps in environmental cleaning, urinary catheter management, and hand hygiene.

What This Means

  • This large, regional multifacility outbreak demonstrates potential for rapid regional spread of drug-resistant pathogens and was likely fueled by gaps in infection prevention and control practices. NYSDOH has started a regional initiative to disrupt transmission and limit infections.
