The Issue

  • Accurately assessing waning vaccine effectiveness (VE), or declining VE post-vaccination, is crucial for guiding policy and measuring vaccine impact. However, detecting waning is challenging, and statistical approaches have been limited for real-world observational data.

What We Did

  • We applied two survival-based methods primarily used with simulated and trial data, for the first time using pooled surveillance data from 21 countries to evaluate how they compared with traditional approaches in detecting waning VE.

What We Found

  • The survival-based methods accommodated time-varying VE measures, which was more appropriate for detecting waning compared with traditional approaches.

What This Means

  • To accurately detect waning VE using observational data, investigators need to incorporate time-varying measures and covariates to differentiate effects of waning from biases.
