Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes, Heart Disease, and Stroke
About the Program
CDC’s Division of Diabetes Translation (DDT) funds state and local health departments to support programs and activities to prevent or delay the onset of type 2 diabetes and to improve health outcomes for people diagnosed with diabetes.
This 5-year cooperative agreement, which began in October 2018, funds 21 state health departments, large city and county health departments, and a consortia of city and county health departments. Using CDC support, these health departments are working to develop new and innovative approaches to increase the reach and effectiveness of evidence-based public health strategies in populations and communities with a high burden of diabetes, heart disease, and stroke.

Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes, Heart Disease, and Stroke
Strategy A.1: Implement systems to support bi-directional e-referral between health care systems and CDC-recognized lifestyle change programs.
Strategy A.2: Increase enrollment in existing CDC-recognized lifestyle change programs or start and sustain new programs in underserved areas.
Strategy A.3: Implement tailored communication to reach high-burden populations and increase awareness of prediabetes and the National DPP.
Strategy A.4: Support advanced training for lifestyle coaches to strengthen skills needed to engage and keep participants in CDC-recognized lifestyle change programs.
Strategy A.5: Test innovative approaches to get rid of barriers to participation and retention in the National DPP lifestyle change program or DSMES.
Strategy A.6: Establish or expand the use of telehealth for DSMES, the National DPP lifestyle change program, or diabetic retinopathy screening.
Strategy A.7: Increase use of clinical systems and care practices to improve health outcomes for people with diabetes.
Strategy A.8: Increase the use of clinical decision support within the EHR to promote early detection of chronic kidney disease.
