Geographic Patterns: Heart Disease Death Rates for 2017 through 2019 for Adults Aged 65 Years and Older by County. The map shows that concentrations of counties with the highest heart disease death rates – meaning the top quintile – are located primarily in Mississippi, Oklahoma, Arkansas, Louisiana, Alabama, and Guam.  Pockets of high-rate counties also were found in New York, Michigan, West Virginia, Arizona, Utah, western New York, Pennsylvania, Kentucky, Tennessee, Georgia, Missouri, and Nevada.
