GFF Past Recipients
In 2020, the Association of State and Territorial Health Officials (ASTHO) and National Association of City and County Health Officials (NACCHO) awarded CDC funds to 42 community health coalitions across the country as a part of the Improving Social Determinants of Health–Getting Further Faster (GFF) initiative.
The 42 recipients conducted community health pilots to learn which approaches work best to address the social determinants of health (SDOH) in one or more of five domains directly linked to chronic disease:
- Built environment
- Community-clinical linkages
- Food and nutrition security
- Social connectedness
- Tobacco-free policies
The year 1 evaluation [PDF – 1.53 MB] is now available.
CDC awarded funds to ASTHO and NACCHO to lead this project through the Strengthening Public Health Systems and Services Through National Partnerships (CDC-RFA-OT18-1802) cooperative agreement.
