VFC Updates (Mary Huynh and Nancy Fenlon)

  • New "Purpose of Contact" in 2010 VFC Management Survey
    We now have three additional options for defining the purpose of the contact with provider, namely, the secondary education follow-up, tertiary education follow-up, and the AFIX follow-up options. These three visit type options will be added to Table 4 of the VFC Management survey and grantees will need document the # of secondary education, tertiary education, and AFIX follow-up contacts conducted for the next cycle of VFC Management Survey due in 2010.
  • Contact Definitions for Table 4 (also found in the Library in CoCASA)
    If you're a user of CoCASA, the definition of each visit type can be found by holding the cursor over the name of the purpose of contact options under the VFC-AFIX Evaluation – Visit Information Tab. The definitions for the VFC-related contacts can also be found in Appendix 6 of the VFC Operations Guide.
  • VFC Site Visit is selected if the visit is a formal visit to a VFC-enrolled provider site for the purpose of evaluating the provider’s compliance with the VFC program requirements. The VFC Site Visit Questionnaire must be completed to proceed.
  • AFIX Visit is selected if a full AFIX visit was conducted. If the "A" and the "FIX" occurred on separate days, use the date the "FIX" was implemented as the date of contact with provider.
  • Educational Visit is selected when the visit is a special visit to the provider’s office to perform an educational in-service (e.g., to discuss recent changes to the immunization schedule). An educational visit is separate from VFC site visits or AFIX visits, which already incorporate an educational component. Select only if educational visit purpose is not related to an identified VFC compliance issue.
  • VFC Enrollment Visit is selected if the purpose of the visit is to enroll the provider/clinic in the VFC program.
  • VFC Site Visit Follow Up is selected if the visit is a contact made with a VFC-enrolled provider to review progress towards correcting a problem identified during the most recent VFC site visit that was not significant enough to enroll the provider into secondary or tertiary education follow-up.
  • AFIX Visit Follow Up contact should be made with every provider who receives an AFIX visit no more than six months following the Feedback. The purpose of the follow-up contact is to continue the process of encouraging the provider toward quality improvement, based specifically on the findings and discussion from the previous AFIX Feedback.
  • VFC Contact is selected if any contact was made with a provider/clinic between VFC site visits (but not related to the most recent VFC Site Visit) for VFC related issues. For example, if a provider/clinic calls because their office has run out of HPV VIS statements, the visit made to the office to bring additional VIS copies would be considered a VFC Contact.
  • AFIX Contact is selected if any contact was made with a provider/clinic in between full AFIX visits for the purpose of following-up on the implementation of quality improvement activities.
  • VFC Secondary Educational Follow-up is selected if the visit is a follow-up that is part of a Secondary Education program because the provider was identified with VFC compliance issues and was subsequently enrolled in a Secondary Education program. The follow-up contact may be in the form of a site visit or other contacts like phone call, email or fax.
  • VFC Tertiary Educational Follow-up is selected if the visit is a follow-up for a provider who was identified with non-compliance issues and enrolled in Tertiary Education program. The follow-up contact may be in the form of a site visit or other contacts like phone call, email or fax.
  • VFC Contact List
    Please review the list of VFC contacts and notify CDC of any changes.

    CoCASA 4.1 is now available to download. Nathan reviewed the updates and changes from the previous version. These can be seen here: http://www.cdc.gov/vaccines/programs/cocasa/documentation.htm
