Register Intent to Submit EHR Data

Key points

  • Use the National Health Care Survey (NHCS) Registry to report your intent to provide electronic health record (EHR) data if selected for one of our surveys.
  • Registering intent to send data to NHCS can meet one of the requirements for the Medicare Promoting Interoperability Program.

Registering

The National Health Care Surveys are one way healthcare providers and facilities can meet the public health reporting objectives of two Centers for Medicare and Medicaid Services programs. The relevant program depends on the healthcare setting—

  • Promoting Interoperability (PI) Program (formerly known as Meaningful Use) is for eligible hospitals and critical access hospitals
  • Medicare Quality Payment Program’s Merit-Based Incentive Payment System (MIPS) is for eligible clinicians

Registering through the National Health Care Surveys Registry Portal meets these programs' requirements for active engagement under Option 1 (Registration of Intent).

If invited to participate in one of the National Health Care Surveys, eligible providers and clinicians must move to the next stages of active engagement to receive bonus points

  • Registration of Intent, Testing, and Validation (Option 1)
  • Validated Data Production (Option 2)

Provider registration

Individual providers can record their intent to submit EHR data upon request through the National Health Care Surveys Registry.

Steps

  1. Log in to the National Health Care Surveys Registry Portal.
  2. Click the "Add Provider" button from the "Navigate To" drop down menu.
  3. Select "Go."
  4. Complete and submit the Provider Signup form.

Hospital registration

Individual hospitals can record their intent to submit EHR data upon request through the National Health Care Surveys Registry.

Steps

  1. Log in to the National Health Care Surveys Registry Portal.
  2. Click the "Add Hospital" button from the "Navigate To" drop down menu.
  3. Select "Go."
  4. Complete and submit the Hospital Signup form.

Group registration

There are two types of Group Registrations.

Actual Group Registration records an existing group practice's intent to submit data to the National Health Care Surveys Registry. Actual Group Registration requires an National Provider Identifier (NPI) number.

Logical Group Registration allows a user to organize their affiliated Provider, Group, and Hospital Registrations. Logical Group Registration does not require an NPI number.

Both group registrations can generate an Annual Active Engagement Verification Documentation letter for the current calendar year.

Steps

  1. Log in to the National Health Care Surveys Registry Portal.
  2. Click the "Add Group" button from the "Navigate To" drop down menu.
  3. Select "Go."
  4. Complete and submit the Group Signup form.

National Provider Identifier number‎

A valid National Provider Identifier (NPI) number is required for each provider, hospital, and actual group registration.

Verify your NPI number, through the National Plan and Provider Enumeration System NPI Registry or the National Provider Identifier Database.

Completing registration over multiple days

You can complete one or more registrations over multiple days.

You can start a registration and then save it to submit at a later date. Your registration will be in DRAFT status until completed.

All registrations must be submitted by the December 31 calendar year deadline. Once submitted, you will receive an Annual Active Engagement Verification Documentation letter. This will note the date you actively engaged with us for that calendar year. You must fully complete the registration process to successfully enter active engagement Option 1. If you submit more than one registration, you will receive a letter for each registration.

Update registration information

To update your registrant information, follow the steps below:

  1. Log in to the National Health Care Surveys Registry Portal.
  2. View a list of the existing registrations on your dashboard.
  3. Click the "Edit" button on the registration you would like to update.
  4. Modify the displayed information.
  5. Click the "Submit" button to complete the update.
  6. Receive an email confirmation of the update.

Update often‎

Please update your provider, group, and hospital registrations as frequently as possible in the National Health Care Surveys Registry Portal.

Transfer registration

The Transfer Ownership function will transfer ALL registrations from you to the new owner. To transfer registrations to another owner, that person must have an ACTIVE account with NO registrations.

Steps

  1. Log in to the National Health Care Surveys Registry Portal.
  2. Click the "Transfer Ownership" button from the "Navigate To" drop-down menu
  3. Select "Go."
  4. Enter the email address of an existing user to receive the registrations.
  5. Click "Submit."
  6. Receive an automated email with an email verification link.
  7. Click the verification link, or copy and paste it into a new browser, to complete the Transfer Ownership process.

Deregistering

If you no longer want to remain in active engagement, you can deregister by withdrawing your registrations from the Registry Portal.

Steps

  1. Log in to the National Health Care Surveys Registry Portal.
  2. View a list of the existing registrations on your dashboard.
  3. Click the "Withdraw" button to the right of the registration you would like to deregister.
  4. Receive a confirmation email.
