Racial and Ethnic Approaches to Community Health (REACH), 2023 – 2028

Forty-one organizations received funding to implement CDC’s Racial and Ethnic Approaches to Community Health (REACH) program. In the five-year program (CDC-RFA-DP-23-0014), the recipients will put into action evidence-based strategies for:
- Food and nutrition security through the promotion of food service and nutrition guidelines and the expansion of existing fruit and vegetable vouchers incentive and/or produce prescription programs
- Safe and accessible physical activity
- Continuity of care in breastfeeding support
- Early care and education (ECE) settings
- Family healthy weight programs
- Tobacco prevention and control policies
- COVID-19, flu, and other routine vaccines
The strategies aim to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease. REACH programs work specifically among African American, Black, Hispanic, Latino, Asian American, Native Hawaiian/Other Pacific Islander, American Indian, and Alaska Native populations.
