CDC requires funded program recipients to submit a variety of programmatic and financial reports at specific times during the life cycle of a grant award (cooperative agreement). All reports must be accurate, complete, and submitted on time.

Reporting Guidance Program for PS18-1807

  • Year 6 Work Plan Status Report #2, HIV Program Review Panel – Chair Summary Sheet, and Success Story are due October 29, 2024.
  • COVID 19 Supplement 1 & 2 Final Report due October 29, 2024.
  • Health Equity Supplement Final Report is due October 29, 2024.
  • Final Federal Financial Report is due October 29, 2024.

Reporting Guidance for Program DP24-0139

  • Annual Performance Report (Year 1 Work Plan Status Report #1, Year 2 Work Plan, and Year 2 Budget), and HIV Program Review Panel-Assurance of Compliance Form (panel members) are due April 1, 2025.
  • Program Evaluation Reporting System (PERS) Data for Reporting Period 1 (August 1, 2024 – July 31, 2025) is due October 1, 2025.  PERS-2 (cdc.gov).
  • Year 1 Work Plan Status Report #2, HIV Program Review Panel-Chair Summary Sheet, and Success Story are due October 1, 2025.
  • Final Federal Financial Report is due October 29, 2025.
