Purpose

The checklist below is a reference for evaluators with tips and prompts. The checklist can help ensure critical steps have been included throughout the evaluation lifecycle.

Step 2: Describe the Program

Step 2 of the CDC Program Evaluation Framework involves developing a program description that identifies the outcomes the program intends to achieve and the key activities that are expected to lead to those outcomes. This often includes the development of a logic model or roadmap, in which interest holders may be engaged.

Applying the Cross-Cutting Actions and Evaluation Standards

As with the other evaluation framework steps, consider how to apply the cross-cutting actions and evaluation standards when developing your logic model and narrative. Refer to Table 3 in the CDC Program Evaluation Framework, 2024 to assess how these cross-cutting actions and standards are applied to the program description.

Organize relevant background materials to support the development of the program description, including:

  • Mission and vision
  • Goals and objectives
  • Proposal or existing program description
  • Strategic plan or other planning documents
  • Program communications plans
  • Existing or previous logic models
  • List of current performance measures

Determine whether you will start with the narrative description or the logic model (based on interest holder preference as well as complexity of the program):

Develop the program logic model or program roadmap:

  • Identify the key components of the logic model: inputs, activities and outcomes.
    • Arrange components in a logical flow, using if-then statements.
    • Clearly depict how the activities will lead to the outcomes via a visual representation of those relationships.
  • Consider if the activities are aligned with the outcomes: Are the outcomes aligned with the program goals? How do the activities lead to outcomes?
  • Collaboratively engage with your interest holders to develop the logic model or narrative description (whichever you develop first)
  • Share the draft logic model with interest holders for feedback and discussion before finalizing

Develop a narrative description to accompany the program logic model or roadmap:

  • Describe the program purpose and context
  • Describe the problem or opportunity the program addresses, e.g., the need
  • Describe what is included in each component of the logic model with examples (i.e., inputs, activities, short-term, Intermediate, and long-term outcomes)
    • Inputs: Define the resources needed for the program (e.g., personnel, funding, materials)
    • Activities: Outline the specific actions or interventions the program will implement
    • Outcomes: Specify the expected changes or benefits resulting from the program categorized by short-term, intermediate, and long-term
  • Provide a rationale for why interest holders expect certain components to lead to desired outcomes (the theory of change), citing evidence when available
  • Identify resources needed for the evaluation, e.g., the input(s)
  • Describe the program's stage of development
  • Identify the potential barriers to success and limitations (contextual factors)
