Understanding Eligibility for CDC-RFA-DP21-2109

Section 1 – General Eligibility

  1. Are you one of the following?
    1. State or local government or their Bona Fide Agent (including the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau)
    2. American Indian/Alaska Native tribal government (federally recognized)
    3. American Indian/Alaska Native tribally designated organization
    4. American Indian/Alaska Native Urban Indian Center
    5. Health Service Provider to federally recognized tribes
    If yes, continue to step 2. If no, you are not eligible to apply for this NOFO.
  1. Does your organization have at least one year of experience working with Community Health Workers (CHW)?
  2. If yes, continue to step 3. If no, you are not eligible for any component of this grant.
  1. Has your organization worked with CHWs for at least one year in your catchment area, but in a limited capacity? Meaning, the programs your organization have implemented engaging CHWs focus on only one or a few diseases or provide advice and guidance to connect community members with available clinical and social resources. Can you provide documentation of such programs?
  2. If so, you can consider an application under Component A. See Section 2 below for more information.If your organization has greater experience than what has been described, continue to step 4.
  1. Has your organization worked with CHWs for at least three years in your catchment area? Have the programs your organization implemented included: a) engaging CHWs to identify and enroll community members in applicable health and social service programs, b) engaging CHWs to coordinate healthcare and provide referral and follow-up services, c) engaging CHWs as advocates for community members needing language assistance or other health system support, d) working directly with public and private health care payers to officially incorporate CHWs into healthcare teams? Can you provide documentation of improvements in the health status of the communities served within your catchment area? Can you describe an evaluation and quality assurance system your organization uses to continually improve your CHW efforts?
  2. If so, you can consider an application under Component B. See Section 3 below for more information. If your organization has less experience than what has been described or is unable or provide documentation, then go back to step 3.

Section 2 – Component A Eligibility

  1. Does the catchment area your organization plans to work within have a population of up to 50,000 people? If so, you can apply for up to $350,000 in funding; continue to Chart 2, Section 1 to determine what strategies you must demonstrate in your application. If the population you plan to work with is larger than this, continue to step 6.
  2. Does the catchment area your organization plans to work within have a population between 50,000 and 200,000 people? If so, you can apply for up to $600,000 in funding; continue to Chart 2, Section 1 to determine what strategies you must demonstrate in your application. If the population you plan to work with is larger than this, continue to step 7.
  3. Does the catchment area your organization plans to work within have a population greater than 200,000 people? If so, you can apply for up to $1 million in funding; continue to Chart 2, Section 1 to determine what strategies you must demonstrate in your application.

Section 3 – Component B Eligibility

  1. Does the catchment area your organization plans to work within have a population of up to 200,000 people? If so, you can apply for up to $1 million in funding; continue to Chart 2, Section 2 to determine what strategies you must demonstrate in your application. If the population you plan to work with is larger than this, continue to step 9.
  2. Does the catchment area your organization plans to work within have a population between 200,000 and 600,000 people? If so, you can apply for up to $2 million in funding; continue to Chart 2, Section 2 to determine what strategies you must demonstrate in your application. If the population you plan to work with is larger than this, continue to step 10.
  3. Does the catchment area your organization plans to work within have a population of greater than 600,000 people? If so, you can apply for up to $3 million in funding; continue to Chart 2, Section 2 to determine what strategies you must demonstrate in your application.
  4. Any applicant applying for Component B is also eligible to apply for Component C. See Section 4 if interested in applying for Component C.

Section 4 – Component C Eligibility

  1. Has your organization decided to apply for Component B, but also has an idea for an innovative project that will address policy, systems, or environmental changes in order to further train, deploy, and engage CHWs to respond to COVID-19, address health disparities exacerbated by COVID-19, and create resilient communities better prepared to respond to future public health crises?
  2. If yes, you are eligible to apply for Component C, an additional $2 million on top of Component B funding, to implement this innovative project. Continue to Chart 2, Section 3 to see what is required in a Component C application.

    If no, then return to Section 3 to review the information on Component B eligibility.
