Host Site Application Process
The host site application period is now closed.
Prospective applicants can apply to host an associate through the Fellowship Management System. All applications must be completed in full and submitted online to be considered for this opportunity.
The PHAP host site application consists of two components: a completed application and a signed PHAP Host Site Agreement to Detail. PHAP recommends that prospective host sites visit this website often for updates.
2013 Timeline

timeline and are subject to change from year to year.)
| March | The host site application period opens. |

| April | Host site application period closes. Applicants may be contacted for additional information. Communications will be initiated by email, and further information may be completed with PHAP staff via phone, email, or in person. |
| May | Host site applicants are notified of their selection status. |
| June | Associates are chosen and matched with selected host sites. PHAP will inform both the host sites and the associates about matches. |
| July | Associates begin host site assignments. |
| August | Associates will attend the CDC/PHAP orientation at CDC headquarters in Atlanta, Georgia. |
