Host Site Application Process

The 2014 host site application period is now open closed.

Prospective applicants can apply to host an associate through the Fellowship Management System. All applications must be completed in full and submitted online to be considered for this opportunity.

The PHAP host site application consists of two components: a completed application and a signed PHAP Host Site Agreement to Detail. PHAP recommends that prospective host sites visit this website often for updates.

2014 Timeline
(Please note: the dates below provide an example timeline and are subject to change from year to year.)

January The host site application period opens January 21.
March Host site application period closes March 5. Host site applicants may be contacted for additional information.
July Host site assignments and associates are chosen and matched. PHAP will inform both the host sites and the associates about matches.
October Associates begin host site assignments.
November Associates attend the CDC PHAP orientation at CDC headquarters in Atlanta, Georgia.
