Application Process

The host site application period for 2012 is now closed.

Those that submitted an application to be a host site for the PHAP class of 2012 are agreeing to the terms listed in the PHAP Host Site Agreement to Detail.  If your host site is selected to host an associate, the PHAP Host Site Agreement to Detail [PDF - 326KB] must be signed by the appropriate staff for your health department and submitted to PHAP (phap@cdc.gov) by July 5, 2012

PHAP recommends that prospective host sites visit this website often to find updates, including when the host site application opens next year and the required supporting documents.  The PHAP Host Site Application typically consists of two components: an application and a signed PHAP Host Site Agreement to Detail. 

2012 Timeline (Please note: the dates below are subject to change)

February

The host site application period opens.

March

The host site application period closes and applications are reviewed.

May

Host site applicants are notified of their selection status.

June

Associates are selected and matched with host health agencies.  PHAP will inform both the host site and the selected associate of the match.

July

Associates attend a CDC/PHAP orientation in Atlanta, Georgia. It is anticipated that PHAP associates will begin their host site assignments on July 30, 2012.
