Eligibility

You must be

A medical or veterinary student in your 2nd or 3rd year when you apply
Enrolled in a school accredited by the
- Liaison Committee on Medical Education
- American Osteopathic Association
- American Veterinary Medical Association
Covered by medical insurance during the fellowship
A U.S. citizen or permanent resident
Able to attend the fellowship orientation, January 10–13, 2013 (unless you are out of the country on your field assignment)

January 16, 2012	Application period opens
February 17, 2012	Application deadline (midnight, EST) Postmark date for supporting documents
April 16, 2012	Selected applicants notified
July 1, 2012	Fellowship year begins Fellows complete assignments at their school
September 1, 2012	Projects begin
January 10–13, 2013	Fellowship orientation at CDC headquarters in Atlanta, Georgia
June 30, 2013	Fellowship year ends

Applications for 2013 will be accepted in January. Apply online. Additional supporting documents are required.

Supporting documents must be postmarked by the deadline.

Submit 2 letters of recommendation
Select individuals familiar with your academic or professional activities who can comment on the traits and accomplishments that distinguish you as a good candidate for this fellowship
Provide them with a copy of the Hubert Fellowship Letter of Recommendation (82 KB; 2 pages)
