About CDC Funded Sites

image of us map

The inaugural partners of the National Diabetes Prevention Program were the Y (also known as YMCA of the USA) and UnitedHealth Group (UHG). To build a network of evidence-based structured lifestyle programs for those at high risk of developing type 2 diabetes, CDC awarded a grant to the YMCA of the USA that initiated some of the first community-based sites to carry out the diabetes prevention intervention. These sites will continue to be supported by United HealthCare (UHC) through reimbursement to the Y or participants that are insured by UHC. CDC is enthusiastic about other organizations becoming involved in the National Diabetes Prevention Program.
