Two forms of documentation are required after the implementation of the educational activity. These include the Postactivity Documentation Report (provided by your CE consultant) and the Satisfaction Survey.

Postactivity Documentation Report

The CE Office is required to maintain a database and make an annual report summarizing all CE activity. Therefore, you must complete Post activity Documentation Report(s) providing us with the following:

  • Total number of participants
  • Total number of credits/contact hours awarded by type
  • For CNE, a list of names, addresses, and the number of contact hours earned for each person awarded credit
  • For CECH, a list of names, CHES numbers, and the amount of contact hoursfor each person awarded credit
  • For CPE, a list of names and the number of contact hours earned for each person

For live educational activities

The Postactivity Documentation Report (provided by your CE consultant) must be returned within 45 days of the completion of the educational activity.

For enduring materials

A Postactivity Documentation Report (provided by your CE consultant) must be submitted to the CE Consultant quarterly throughout the time period for which credit is provided. If your educational activity is linked with CDC/ATSDR's Training and Continuing Education Online system, you may request the data needed to complete these reports at the end of each quarterly reporting period. Reports are due January 15th, April 15th, July 15th, and October 15th.

Satisfaction Survey

The Satisfaction Survey is an online survey of your experience with the CE Office and the CE proposal process. It should be completed by the CDC contact person. In addition, anyone else who was involved in the process is invited to complete a survey.
