Program Evaluation
Indicators for School Health Programs
State, territorial, local education agencies, and tribal governments
(SEAs, TEAs, LEAs, and TGs) funded by CDC's Division of Adolescent
and School Health (DASH) under Program Announcement 03004 (March
1, 2003–February 29, 2008) or Funding Opportunity Announcement
DP08-801 (March 1, 2008–February 28, 2013) to provide asthma
management, HIV prevention, or coordinated school health programs
(CSHP) are required to complete the Indicators for School Health
Programs (Indicators). The Indicators provide process evaluation
data for activities conducted by DASH funded partners. This page contains
resources for DASH funded partners and others interested in partners'
activities.
