Health Equity in Action
Health Equity in Action uses best practices to spotlight projects that align with CDC’s CORE (Cultivate, Optimize, Reinforce, Enhance) commitment and shares the lived experiences of communities advancing health equity. CORE is the framework for embedding health equity into CDC’s science and interventions, and diversity, equity, inclusion, and accessibility (DEIA) into CDC’s workforce, culture, and infrastructure.
The project snapshots below serve as examples to highlight CDC work and external CDC-funded projects that center around engaging and mobilizing partners with culturally appropriate strategies to reduce health disparities and advance health equity.

To improve adult vaccine confidence and coverage for COVID-19 and other vaccines among three disproportionately affected populations – people experiencing homelessness, people with substance use disorders, and people who engage in sex work – by hiring and supporting Vaccine Ambassadors (community health workers focused on vaccinations) through community health centers.
