Social Determinants of Health

Social determinants of health are factors in the social environment that contribute to or detract from the health of individuals and communities. These factors include, but are not limited to the following:

	Spotlight 19th National Conference on Chronic Disease Prevention and Control This year’s conference theme is Health Disparities: Progress, Challenges, and Opportunities. The conference will have numerous sessions on topics that directly or indirectly relate to social determinants of health.

• Socioeconomic status
• Transportation
• Housing
• Access to services
• Discrimination by social grouping (e.g., race, gender, or class)
• Social or environmental stressors

In October of 2003, the Centers for Disease Control and Prevention sponsored a 2-day forum, Addressing Social Determinants of Disparities in Health: Learning from Doing

• The forum highlighted community interventions to reduce health disparities by addressing social determinants of health.
• Nine projects were presented and cross-cutting themes were discussed.
• Participants included community, media/communication, public health practice, academia, and funding organizations.
