The Statewide Health Improvement Partnership (SHIP) is an innovative effort to reduce the risk factors that lead to chronic disease through locally driven changes to policies, systems, and the environment. This work is only possible through strong relationships with partners across the state. This map portrays the wide reach that SHIP partners have across Minnesota.

The map to the left depicts Statewide Health Improvement Partnership (SHIP) partners in Minnesota over SHIP regional boundaries (defined by groups of counties that share one or more community health boards). The series of three vertically stacked choropleth maps depicts the number of partners working on specific community-level SHIP strategies within each SHIP grantee region.

Major Findings 

This map represents the locations of SHIP’s 3,000 partner sites that are working across the state to improve local access to healthy foods, increase opportunities for physical activity, and create tobacco-free environments. Partner locations are often clustered in cities and small towns, but in rural areas these partners often serve wide geographic areas.

This map series has been used to communicate the reach of SHIP work and to inform local and state level planning and technical assistance provision. Other maps in the series have been shared with local public health leaders, legislators, and other local decision makers to demonstrate the breadth of SHIP’s work in communities across the state.
