Impact Statement

The social determinants of health are conditions in the environment in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality of life outcomes and risks. This map series details 5 social determinants and where they are most burdensome in Florida. When this data is overlaid with heart disease death rate data then it is easy to see that often those counties with poor outcomes on the measures associated with the social determinants also experience an increased amount of deaths associated with heart disease.
