Answers to most questions about the CLIA program can be found on the CMS CLIA website or by emailing CMS. Since CDC frequently receives CLIA-related questions,  we provide links to information about the most common topics.

Certificates

Find out how to apply for a certificate.

Get answers to questions about W-9 forms, certificate status, renewal fee coupon, updating certificate information, regulations, and licensure from your state agency.

Pay your CLIA laboratory user fee online or mail your payment to the address on the coupon by U.S. Postal Service only.

Look up your CLIA certificate expiration date using your certificate number or laboratory name.
