REACH Funding Opportunity Announcement (FOA)
REACH projects are funded to implement sustainable practice- and evidence-based strategies impacting health disparities. Applicants should be able to demonstrate national or multi-state reach through local partnerships or program activities. At least 75 percent of the funds awarded through this FOA will be disseminated to local partners.
Recipients will address health disparities in racial and ethnic groups, including but not limited to: African American or Black, Hispanic or Latino, American Indian or Alaska Native, Asian American, and Native Hawaiian or Other Pacific Islander. Recipients may choose to implement strategies that address disparities in cardiovascular disease, diabetes, breast and cervical cancer, infant mortality, asthma, or child and adult immunization.
CDC expects to make 6–10 cooperative agreement awards under the REACH FOA, with successful applicants announced in September 2012.
